Our secured forms are within the Patient Portal below. If you are new to our office, schedule online for your appointment first to create a profile in our system. Afterwards return to this page and click on the "Register" below to create your own username and password to your own portal. Once submit you will get a verification that can be either sent to the phone number or e-mail you provided during the scheduling of your appointment. Once logged in, you can click on the tab "Patient Forms" and can update your Demographics, Insurance information, Reason for Your Visit, and most importantly your Medical History. If you've been with us before, you can just click the "Register" tab to create your own login to update Demographics, Insurance, Reason for Visit, and update Medical History (this needs to be done for each exam year).
Appointments booked online here are locked in, unless we contact you otherwise. Thank you for being flexible. If you would like to change the appointment can e-mail or call the office.
Please be aware that appointments are for exam only and not for buying glasses or contacts. E-mail us if you would like to schedule to look at glasses.

Use it or Lose it Flexible Spending Account (FSA) Dollars Remember to use all of your flexible spending account dollars by December 31st. Unused FSA dollars are lost at the end of the year.
